the sickest or most disturbing films you have seen?
mine would have to be...
ichi the killer, with the beatings and torture and self mutilation and hot oil being poured on a man hanging on hooks, great story though
erm another japanese one was old boy

ive seen loads of horror ones but are there any like ichi?

You mention that you don't want gore but I have to tell you the sickest movie of all time fits in that category. Hands down, nothing will prepare you for August Underground: Mordum!
That is by far the most vile, wretched movie that has ever come out. It is a sick flick about 3 people going around and murdering anyone that they see. This flick is actually the second part of a 3 part series. You don't have to watch them in order though, they are all about the same thing primarily.
August Underground 1 - The story follows 2 guys around as they humiliate and violate mostly women but there are a few guys thrown into the mix.
August Underground 2 - Mordum - 2 new characters are added in this story, a brother and sister join our first killer/torturer from the first flick and go on another killing spree. Absolutely the sickest thing you'll ever see!
August Underground 3 - Penance - This one has the original character with his girlfriend as they complete the cycle of murder and mayhem.
It's shot to be like a real snuff movie, if this isn't your style, Salo would definitely fit the bill that you are trying to fill. Great sick flick and hard to stomach but not really super gory or anything just really a hard view. There is a ton of degradation and humiliation on children in this flick. It gets progressively worse as the film goes through it's "rings."
Portrait of a Knife - This movie is also way up on the throw up meter. It's over 4 hours long and it shows the real life tortures of a Japanese Death Camp in World War 2. This is a very hard watch and can drag at times but it will have you cringing in some scenes. Experiments are shown in graphic detail and while some scenes aren't always depicted just knowing what is going on can really mess with you. It starts off slow but about an 45 minutes into it you start getting into the experimentation. A very hard one to watch.
Cannibal Holocaust - Many say this is the sickest horror film ever shot but in reality now it seems a little behind the time. There are abortions, decapitations, cannibalism and more in graphic detail as well as a lot of animal torture too. This is considered the king of the sicko horror/cannibal/jungle films that came out in the late 70's. They have some good effects for the time as well. The film centers on an expedition that goes out to find a group of filmmakers that have vanished. They find rolls of their film and then go back to America to "view" what happened. That's when the action starts.
Aftermath - This is the king of the sicko's in other people's minds too. It's a 30 minute film but it packs a wallop and doesn't let go until the last shot on your screen fades away. There is no dialogue but it revolves around 2 morticians that are doing their job, well one of them is and the other just seems a little off. Wait to see just how off he really is. The music and photography is absolutely amazing but it will really throw you for a loop when you watch the visuals. The music is calming but the visuals are visceral. Great stuff.
I hope some of these you'll investigate. While I'm at it one Japanese flick that gets overlooked is Evil Dead Trap about a late night talk show host that receives a tape in the mail and decides to investigate the origins. Bad mistake and a very well shot movie. Don't bother with Evil Dead Trap #2 though it is definitely an inferior product.
Also, who can miss Rikki Oh! This one is on the violent level of Ichi and s probably the closest that you'll find to Ichi. It features tons of torture and outlandish gore. Those are a few that I could come up with off the top of my head anyway. Enjoy!
